Prostate specific antigen (PSA) is a protein produced by the prostate, a small gland that encircles the urethra in males and produces a fluid that makes up part of semen. Most of the PSA that the prostate produces is released into this fluid and the rest is released into the bloodstream. The PSA test is used as a tumor marker to screen for and to monitor prostate cancer. Elevated levels will likely be followed up with a free PSA test. This test should be used for routine screening. In addition, the total PSA test is ordered at regular intervals during treatment of men diagnosed with prostate cancer. The American Cancer Society (ACS) recommends that screening should take place at age 50 and every year thereafter for men with PSA levels of 2.5 ng/mL or higher and every 2 years if the PSA level is less than 2.5 ng/mL. The ACS also recommends that men at age 45 should be tested if they have a 1st degree relative diagnosed with prostate cancer at an early age (younger than 65) and men at age 40 should be tested if they have more than one 1st degree relative diagnosed with prostate cancer at an early age.

Preparation

Do not perform test after a transrectal biopsy or digital rectal exam (DRE). Avoid ejaculation for 24 hours before sample collection as it has been associated with elevated PSA levels.
