Testosterone is an androgen steroid hormone. It is the primary sex hormone in males but it is present in females as well. It is secreted by the testes in men and low levels are secreted by the ovaries in women. In addition, small amounts are secreted by the adrenal gland in both men and women. Testosterone is typically attached to a protein in the bloodstream called bound testosterone. Free testosterone is when the hormone is unattached. A total testosterone measurement measures the combination of both free and bound testosterone. Where there is an abnormal rating, the typical follow up test is free testosterone which is included. Thus, this test measures both Free and Total testosterone. Free testosterone typically gives more accurate clues for diagnosis. Total testosterone measures testosterone that is bound to proteins in the blood using liquid chromatography and tandem mass spectrometry (LC/MS/MS) in cases of very low concentrations of testosterone. The advantage of using LC/MS/MS for measurements in women and children is it can achieve higher sensitivity and specificity. This test is the gold standard for small molecule determinations. Abnormal levels of testosterone are present in conditions such as delayed puberty, decreased sex drive, erectile dysfunction, testicular tumors, pituitary disorders, infertility in men and women, hirsutism (excessive hairiness in women) and virilization (development of male traits in women).
